Gynecomastia Surgery Scars: How to Minimize Them

While gynecomastia surgery does involve incisions, an experienced surgeon can place them strategically so that gynecomastia surgery scars are minimal, well-concealed, and fade significantly over time.

Where Are Gynecomastia Surgery Scars Located?

Most gynecomastia procedures involve a combination of gland removal and liposuction. The location and size of the scars depend on the technique used and the severity of the condition.

Periareolar incision
Surgeons most commonly place the scar around the lower half of the areola. This is where the surgeon removes the glandular tissue. The natural transition between the darker areola and lighter chest skin helps hide the scar.

Liposuction access points
If liposuction is used, small incisions (typically 3 to 5 mm) are made along the outer edge of the chest or near the armpit. These heal well and often become nearly invisible over time.

Extended incisions
For men with more severe gynecomastia or excess skin, longer incisions may be required. These may be located beneath the breast fold or along the side of the chest. While more visible, they are necessary for cases involving sagging skin or massive weight loss.

What Do Gynecomastia Surgery Scars Look Like Over Time?

Scars go through several healing phases:

Initially, the scars may look pink or red and feel slightly raised
Over the next several months, they begin to flatten and lighten in color
By 6 to 12 months, most scars are flat, faint, and difficult to see at normal distance

Healing depends on several factors, including skin tone, age, genetics, and how well you follow aftercare instructions.

How to Minimize the Appearance of Gynecomastia Scars

Keep incisions clean and dry in the early stages of healing

Avoid sun exposure

Can Scars Be Removed Completely?

While scars can’t be removed entirely, most gynecomastia patients find their incisions heal into thin, nearly invisible lines. If a scar becomes raised, discolored, or bothersome, treatments like laser therapy or microneedling can improve the appearance further.
